Facets of facet joint interventions

This article discusses fluoroscopic and CT-guided intraarticular FJ injections, medial branch (MB) radiofrequency ablation (RFA), and lumbar facet synovial cyst (LFSC) aspiration, rupture, or fenestration.

Lumbar zygapophysial (facet) joint injections
The Spine Journal, 1995The lumbar zygapophysial joints are a potential cause of back and lower extremity pain. Absolute diagnosis of lumbar zygapophysial joint-mediated pain is based on selective analgesic injections of these joints or their nerve supply. The therapeutic role of zygapophysial joint injections is controversial.

In 1933, Ghormley named the spectrum of symptoms arising from its pathological involvement as “facet joint syndrome” (FCS). FCS represents ≤15% of chronic pain in the lower back.

The facet joints, as true synovial joints, were first postulated as an etiology of low back pain and sciatica more than 90 yr ago (1–3). It was not until 1933, however, that the facet joints themselves were reported to be the source of the back pain, at which time the term “facet syndrome” was coined by Ghormley (4).
