Fetus in fetu is a rare cause of intra-abdominal mass. The presence of vertebral and skeletal axis differentiates it from a teratoma. We report an unusual case of two well developed fetuses in the retroperitoneal area of a neonate delivered at term.

Fetus in fetu represents a parasitic twin included within the abdomen of its bearer as a result of embryonal developmental aberration. A distinction between rare cases of fetal inclusion and much more frequent teratomas should be made. Morphologically, fetus in fetu shows a high degree of vertebral organization.

Laparotomy revealed an anencephalic fetus within an encapsulated retroperitoneal mass. Pathologic examination confirmed rib formation with a primitive vertebral column with bone marrow. There was no other organogenesis.
