The intact fibula

The notes and radiographs of 281 adult tibial shaft fractures were reviewed. In 17 per cent there was an intact fibula. Delayed union rates were lower when the fibula remained intact, thus confirming that this is a good prognostic indicator. This fracture is caused by less severe trauma than fractures where both bones are broken.

Fibular hypoplasia occurs in differing degrees of severity with fibular hemimelia as its most severe form. Normally the distal epiphyseal plate of the fibula is at the same level as the distal end of the distal tibial epiphysis, whereas the tip of the proximal fibular epiphysis is level with the proximal tibial epiphyseal plate.

We report four patients with unilateral bowing of the lower leg, affecting only the fibula. The bone is too long with anterolateral curvature of the distal third. Because of its regressive course and the absence of cutaneous involvement, this newly described entity can be distinguished from other forms of bowing of the leg.
