Catheter Ablation: Evolution and Efficiencies

open access: yesJournal of Cardiovascular Electrophysiology, EarlyView.
ABSTRACT The present review includes a discussion the goals and pitfalls of efficiency in the medical profession, a historical perspective on the evolution of catheter ablation in the field of electrophysiology (EP), and the data regarding the relationship between efficiency and outcomes in the EP lab. Focus on efficacy is critical to maximize resource

Liability, insurance and defensive medicine: new evidence [PDF]

open access: yes
For the first time, we test for effects of liability on hospital care using measures of current perceptions of litigation risk at hospital level; in particular, the risk-sharing arrangements agreed between hospitals and their insurers.

Intravascular Lithotripsy to Facilitate Extraction of Very Old Cardiac Implantable Electronic Devices Leads

open access: yesJournal of Cardiovascular Electrophysiology, EarlyView.
ABSTRACT Background One of the challenges encountered when extracting transvenous leads with long dwell times is the presence of dense calcifications encasing the leads. This is the most likely reason for failure to advance a laser extraction sheath.

Real‐Time 3D Mapping Corrects Fluoroscopic Misperception During Pulmonary Vein Isolation by Pulsed Field Ablation

open access: yesJournal of Cardiovascular Electrophysiology, EarlyView.
In a three‐phase comparison (fluoroscopy‐only, 3D‐blinded, and real‐time 3D‐EAM), 23.8% of blinded procedures missed at least one pulmonary vein. Real‐time 3D mapping eliminated missed veins and reduced PVI applications. 3D‐EAM, three‐dimensional electroanatomical mapping; CT, computed tomography; LA, left atrium; PV, pulmonary vein.

Comparison of Integrated 3D Electroanatomic Mapping and Conventional Mapping Systems for Pentaspline Pulsed Field Ablation of Atrial Fibrillation

open access: yesJournal of Cardiovascular Electrophysiology, EarlyView.
ABSTRACT Introduction Pulsed field ablation (PFA) is an established therapy for atrial fibrillation (AF), yet optimal mapping strategies during pentaspline PFA remain uncertain. We compared procedural characteristics and material costs of an integrated 3D electroanatomic mapping (EAM) platform with conventional mapping systems. Methods and Results In a

Characterization of AV Nodal Left Inferior Extension by Use of High‐Density Mapping and Voltage‐Time Relationship

open access: yesJournal of Cardiovascular Electrophysiology, EarlyView.
High resolution voltage LA map is shown with Ripple mapping during sinus rhythm prior to atrial fibrillation ablation (panel A‐D, top row Left Anterior Oblique view, bottom row Left Anterior Oblique Caudal view). Chamber activation is visualized by white Ripple bars, proceeding from the septum and septal roof (panel A), spreading clockwise and ...
