Abstract Two cases of the carcinoid syndrome with valvular disease of the heart showed a reductior in arterial oxygen saturation due to right to left shunt. In one case, necropsy revealed a functionally patent foramen ovale as the cause for the shunt; in the other case a presumptive clinical diagnosis of patent foramen ovale was made.

Hypoxemia can be an early life-threatening complication of orthotopic heart transplantation. Commonly, hypoxemia after orthotopic heart transplantation is due to pulmonary hypertension or pulmonary complications. Rarely, structural defects either in the donor or recipient heart can lead to life-threatening hypoxemia.

We report a case of a 58-year-old man with profound hypoxemia after orthotopic cardiac transplantation for end-stage ischemic cardiomyopathy. Transesophageal echocardiography demonstrated the presence of a patent foramen ovale in the recipient atrial cuff.
