Recurrent Fungemia Due to Candida auris

We present a case of recurrent multidrug-resistant Candida auris (C. auris) in a patient who required multiple hospitalizations. The patient's case was complicated by interval admissions to the intensive care unit for septic and hypovolemic shock for 12 months to manage C. auris fungemia.

内科領域にとって,カンジダの深在性真菌症を診断することは極めて難しい.入院例の喀痰培養で,カンジダは分離菌の15%程度を占めているが,これらの多くは口腔内からの分離菌と考えられ,肺真菌症と考えられるのは極く少数例である.むしろ,アスペルギルスのような病原性の明らかな真菌では,臨床所見,画像所見とともに喀痰や気管支洗浄液でアスペルギルスを分離することにより,診断は容易である.カンジダの深在性真菌症を確定診断するためには,剖検などによることが多いが,剖検の病理所見でもカンジダとトリコスポロンを区別することが困難なことが多い.また,血液よりカンジダを分離した真菌血症の症例でも剖検では,カンジダの深在性真菌症は認めず,逆に酵母様真菌による深在性真菌症では血液よりカンジダを検出しない.今回 ...
