While distal fusion to LTV‐1 on upright radiographs yields satisfactory outcomes in a certain proportion of AIS patients, we hypothesized that the relaxed spinal posture in supine position could facilitate a more conservative fusion strategy. This study therefore investigated the utility of supine imaging for LIV selection in Lenke‐5 AIS.

A two‐incision MI‐TLIF showed comparable outcomes to the traditional four‐incision approach in single‐level patients. Excessive pursuit of smaller or fewer incisions demonstrated no significant short‐term advantage. Reduced exposure may still help minimize wound complications in vulnerable groups, warranting further targeted follow‐up.
