Culicoides hypersensitivity (CH) is a strongly pruritic dermatitis of horses, characterised histologically by mixed perivascular to diffuse cellular infiltrates of mononuclear cells and eosinophils, and caused by helper T cell (Th)2, immunoglobulin (Ig)E‐mediated reactions to antigens present in the saliva of biting Culicoides midges.

In a register‐based study of 22 586 Danish patients with colorectal cancer who had intended curative surgery, a low incidence of metachronous peritoneal metastasis (M‐PM) was found. Risk factors for M‐PM were advanced T and N status and right‐sided colonic tumour.

Moistening surgical wounds with tranexamic acid 25 mg/ml before closure is a simple, low‐cost intervention that reduces bleeding by one‐third and may reduce risk of rebleeding. The effect is comparable to intravenous administration, with a reduced risk of systemic adverse events.

Trainee performance in radical gastrectomy and its effect on outcomes
This study of 272 D2 gastrectomies investigated whether trainee involvement was associated with adverse outcomes. No significant difference in morbidity, mortality, operating time, number of nodes resected, hospital stay or predicted 5‐year survival was noted, demonstrating that outcomes are not compromised by supervised trainee involvement.

Annual surgeon‐level metrics demonstrated the greatest variation (fivefold) risking inappropriate target thresholds. Surgeon‐level performance metrics were not associated with survival, suggesting that compound‐level quality assurance provides better overall performance appraisal.
