An 80-year-old diabetic man (type 2) with detrusor underactivity treated by long-term urethral catheterization attended the emergency department with a 4-day history of scrotal swelling and pain and an associated offensive discharge. Physical examination revealed a grossly swollen right hemi-scrotum with gangrene.
