広島大学医学部附属病院皮膚科において経験し, 手術治療を行った女性の外陰部Paget病11例について手術方法の検討を行ったので報告する。腫瘍切除後の欠損に対して薄筋皮弁を中心とする皮弁による再建を行うと, 分層植皮術に比べ粘膜側の切除も比較的十分に行うことができ, また外陰部の機能の温存をはかることもできた。さらに術後の創の管理も簡単であり, 整容的にも満足のいく結果を得た。以上より女性の外陰部Paget病に対しては, 皮弁を中心とした外陰部の再建が非常に有用な方法であると考えられた。腫瘍切除後の欠損に対して選択する手術方法については, その片側の欠損の大きさ, すなわち外陰部の正中線から欠損縁までの距離が6cmまでであれば単純に縫縮することができ ...
