Giant diverticulum of the colon is a rare finding which has not so far been reported in the British radiological literature. A 66-year-old man was referred to hospital with a 24-hour history of abdominal pain and the passage of three melaena stools. Examination revealed some abdominal distension and tenderness, but no masses were felt.

Giant diverticulum of the colon is a rare condition. If left untreated it will enlarge and give rise to symptoms or serious complications so it is best treated electively once the diagnosis is made. Local excision of the diverticulum could be adequate surgical treatment as an alternative to sigmoid colectomy.

The giant diverticulum of the colon is a rare entity first diagnosed by Bouvin and Bonte in 1946. Few cases have been reported in the literature. It is normally located in the antimesenteric border of the sigmoid colon. In most cases it is considered to be an uncommon complication of a common disease: colonic diverticulosis.
