Abstract Aims Most diabetic foot ulcers are caused by tissue stress from being ambulatory in people without protective sensation. These ulcers are suggested to be preceded by local skin temperature increase due to inflammation of the underlying tissue, a so‐called hotspot. Evidence to support this mechanism of ulcer development is meagre at best.

Abstract Cartilage and osteochondral lesions of the first metatarsophalangeal (MTP‐1) joint are characterized by pain during weight bearing and walking. The lesions often require surgical intervention(s). Arthroscopic bone marrow stimulation may be considered the preferred operative intervention for small lesions.

An estimated 40% of the US population have foot problems. Of all patients aged over 50 years, 2.5% report degenerative arthritis of the first metatarsophalangeal (MTP) joint, termed ‘hallux rigidus’. First MTP osteoarthritis is the most common arthritic condition in the foot. Progression of great toe arthritis is associated with pain and loss of motion.
