HbA1c screening for the diagnosis of diabetes
We read with great interest the study by Young et al [1] on the impact of population-level HbA1c screening on reducing diabetes diagnostic delay in middle-aged adults. Based on HbA1c levels among UK Biobank participants aged 40–70 years, 1% of 166,846 participants were found to have undiagnosed diabetes.

Glycated hemoglobin (HbA1c) is an important indicator of glycemic control in diabetes mellitus, based on which important diagnostic and therapeutic decisions are routinely made. However, there are several situations in which the level of HbA1c may not faithfully reflect the glycemic control in a given patient.

Pitfalls of HbA1c in the Diagnosis of Diabetes
Abstract Many health care providers screen high-risk individuals exclusively with an HbA1c despite its insensitivity for detecting dysglycemia. The 2 cases presented describe the inherent caveats of interpreting HbA1c without performing an oral glucose tolerance test (OGTT).

Analysis of a Point-of-Care HbA1c Assay: Is It Time for an HbA1c Error Grid? [PDF]
In an article in Journal of Diabetes Science and Technology, Arnold et al have presented a thorough study of imprecision components for a point-of-care hemoglobin A1c (HbA1c) assay. An interesting and innovative approach is the combination of data from different studies to arrive at a total error estimate.
