Glycaemic control of diabetic patients, assessed by HbA1c measurements over time, reduces the risk of developing long-term complications. But how reliable is the HbA1c value? Are there circumstances in which a measured value should be questioned? This laboratory quiz describes the diagnostic value, interpretation and pitfalls of the HbA1c-assay.

About the HbA1c in the elderly
Gériatrie et Psychologie Neuropsychiatrie du Viellissement, 2016HbA1c product of non enzymatic glycation of HbA increases in relation with the mean blood glucose level during the former 2-3 months. HbA1c levels are correlated with the development of diabetic complications and HbA1c assessment is now the gold standard for evaluation of diabetes control.

HbA1c Below the Reportable Range
Laboratory Medicine, 2021Abstract This case report concerns a 71 year old female patient who had a very low glycosylated hemoglobin (HbA1c) despite having a high level of fasting blood glucose. The patient had a decreased erythrocyte count, elevated red blood cell indices, and a reticulocyte count with no evidence of hemoglobinopathy.

This case report highlights the importance of recognizing potential pitfalls in HbA1c measurement errors and being vigilant in terms of discrepancies between HbA1c estimated from continuous glucose monitoring (CGM) and venous HbA1c measurements. The latter is especially relevant for physicians managing diabetes, as by 2025, 4,400 adults with insulin ...

The interference by HbF on HbA1c (BM Test HbA1c) measurement in enzymatic method
Annals of Clinical Biochemistry: International Journal of Laboratory Medicine, 2015Background High performance liquid chromatography and immunological and enzymatic methods are known as the methods for HbA1c measurement. However, the differences in the principles of the methods may cause slight discrepancies, which become problems especially in the regions where hemoglobinopathies that have high HbF concentrations are commonly seen ...

HbA1c and iron deficiency: A review
Diabetes & Metabolic Syndrome: Clinical Research & Reviews, 2013Conditions that affect erythrocyte turnover influence HbA1c concentrations and the International Expert Committee has warned clinicians to be aware of any conditions that could affect the turnover of red blood cells. Although many forms of anemia are associated with lowering of HbA1c, iron deficiency has been shown to shift HbA1c slightly upward.
