Experience of Coronary Artery Bypass Grafting on the Beating Heart with a Right Heart Bypass System.
心臓後下面領域への冠血行再建を容易にするために右心バイパス(right heart bypass, RHB)を用いた心拍動下のCABGを1999年11月より導入した.その有用性や周術期の問題点について明らかにするため,RHB使用下に行ったCABG 77例(RHB群)と同時期のOPCAB症例(OPCAB群)30例との比較検討を行った.両群の患者背景に有意差を認めず,入院死亡は認めなかった.バイパス枝数はRHB群2.4±0.6本,OPCAB群2.0±0.2本であった(p=0.002).心臓脱転前後のSvO2の変動は横隔膜面領域,後側面領域ともRHB群で有意差をもって良好な値を保つことができた.肺動脈圧の変動は有意差は認められなかったものの,RHB群でその上昇度が少ない傾向にあった ...
