Abstract Foramen secundum defects of the interatrial septum with large left to right shunts may cause congestive heart failure during infancy. In three infants with this lesion, varying in age from 5 days to 2 years, intractable congestive heart failure developed. Case 1 presented with signs of congestive heart failure in the newborn nursery.

Diagnostic value of the first heart sound in children with atrial septal defect

The pre- and postoperative phonocardiograms of 24 children with uncomplicated atrial septal defect (ostium secundum type) were analyzed. The amplitude of the mitral (M1) and tricuspid (T1) components of the first heart sound were measured in each patient at the apex prior to surgery.

Effect of increase in heart rate on interatrial shunt in atrial septal defect

The effect of increases in heart rate by atrial pacing was investigated in 20 children [mean 9 +/- 4 (SD) years] with atrial septal defect. Systemic blood flow increased by 13 +/- 10 and 27 +/- 22% with a 25 and 50% increase in heart rate, respectively. Pulmonary blood flow, however, remained unaltered.
