A case of the HELLP syndrome in a pregnant woman, which was diagnosed initially as obstructive cholecystitis, is presented. The diagnostic difficulties and some management problems are discussed. Practitioners should be aware of the HELLP syndrome and related conditions when caring for pregnant patients.

The HELLP syndrome is a serious complication of pregnancy characterized by hemolysis (H), elevated liver (EL) enzymes, and low platelet (LP) count that occurs in 0.2-0.6% of all pregnancies and in 10-20% of cases with severe preeclampsia and frequently leads to adverse maternal and perinatal outcome.

[The HELLP syndrome].

The HELLP syndrome is a serious complication of pregnancy, found most frequently in conjunction with severe preeclampsia. The incidence of this disease in preeclampsia is between 2 and 12%. The diagnosis is based on typical laboratory findings, i.e. haemolysis--H, elevated liver enzymes--EL and a low-platelet count--LP.
