Imperforate Hymen Causing Hematocolpos and Urinary Retention

Acute urinary retention is rare in the pediatric population and is typically caused by an obstructing entity. It may result from hematocolpos caused by imperforate hymen. Imperforate hymen is rare, with an associated incidence of 1 in 2000 people.

Presented are two cases of imperforate hymen with hematocolpos seen in a pediatric emergency department (ED) during a seven-month period. The first case presented with abdominal pain, urinary obstruction, and constipation on initial visit. The diagnosis was not made on the initial presentation.

Abstract A case of hematocolpos occurring in a 13-year-old girl has been presented. Of interest was the large amount of retained blood which measured 3,300 c.c. Back pressure effects of the urinary system were demonstrated. Cruciate incision provided adequate drainage. Normal resumption of activity followed an uneventful postoperative course.
