A 22-year-old man with sickle cell disease developed a conductive hearing loss as a result of extramedullary hematopoiesis in the right middle ear. This is the first reported case of conductive hearing loss from extramedullary hematopoiesis and only the second report of extramedullary hematopoiesis in the middle ear.

Case 69: Extramedullary Hematopoiesis

A 66-year-old man was found to be anemic and thrombocytopenic during his annual physical examination. Work-up with bone marrow evaluation revealed mild marrow fibrosis with marked megakaryocytic hyperplasia, a finding that is consistent with early myelofibrosis with myeloid metaplasia. The diagnosis was confirmed with cytogenic study of the bone marrow.

Cutaneous extramedullary hematopoiesis.

An asymptomatic skin nodule in a patient with myelofibrosis was shown to consist of extramedullary hematopoiesis. It was a soft bluish nodule that resembled a hemangioma. Microscopically, both erythroid and myeloid cells, but not megakaryocytes, were found.

Cutaneous extramedullary hematopoiesis in myelofibrosis

Erythematous papulonodules that resembled a malignant lymphoma developed in a 62-year-old man. Further examination revealed that he had primary myelofibrosis with cutaneous extramedullary hematopoiesis.
