An Alternative Technique for Hemiarch Replacement Without Using Deep Hypothermic Circulatory Arrest

Seminars in Thoracic and Cardiovascular Surgery, 2017
We describe an alternate technique to perform a replacement of the aortic hemiarch without the need for hypothermic circulatory arrest by axillary cannulation and arch vessel isolation. In 2015, 3 patients underwent hemiarch reconstructions for ascending aortic aneurysms that extended into the arch.

Effects of Residual Arch Tears on Late Outcomes After Hemiarch Replacement for DeBakey I Dissection

open access: yesAnnals of Thoracic Surgery, 2023
Background: This study evaluated the effect of residual arch tears on late reinterventions and arch dilatation after hemiarch replacement for patients with acute DeBakey type I aortic dissection.

Revisiting ascending aortic resection in the elective valve-sparing root replacement: assessing the benefits and necessity of hemiarch replacement at three centres

open access: yesEuropean Journal of Cardio-thoracic Surgery
OBJECTIVES: The aim of this study was to determine the indication and optimal timing for performing a hemiarch procedure in patients undergoing valve-sparing root replacement (VSRR).

Aortic root and hemiarch replacement in osteogenesis imperfecta

Asian Cardiovascular and Thoracic Annals, 2012
We describe a case of osteogenesis imperfecta in a 52-year-old man who underwent successful repair of aortic root dilatation and aortic valve insufficiency, using a bioprosthetic aortic valve anastomosed to an interposition graft.

A new method for graft tailoring in hemiarch replacement

The Annals of Thoracic Surgery, 2004
We describe a method of graft tailoring in hemiarch replacement that can accommodate long elliptical distal anastomosis and the right angle between the arch and ascending aorta without kinks, resulting in a more physiologic shape of the neoaorta. The distal end of the graft is beveled superiorly, to spare the origins of brachiocephalic vessels as in ...

Surgical results of hemiarch replacement for acute type a dissection

The Annals of Thoracic Surgery, 2002
The appropriate surgical strategy for patients with an arch tear in acute type A dissection remains controversial. We retrospectively compared surgical results after hemiarch as compared with transverse aortic arch replacement in patients with an arch tear in acute type A dissection. The records of 88 patients who consecutively underwent graft ...

Computational Analysis of the Mechanical Behaviors of Hemiarch and Total Arch Replacements

Annals of Biomedical Engineering, 2015
Surgery for aortic dissections or aneurysms can be extended into the aortic arch by hemiarch replacement (HAR) or total arch replacement (TAR). Although cardiovascular surgeons have been performing HAR and TAR for decades, the mechanical properties of HAR and TAR are not well understood.

The Bio-Bentall procedure with concomitant hemiarch replacement through a right anterolateral minithoracotomy

The Multimedia Manual of Cardio-Thoracic Surgery, 2022
A right anterior minithoracotomy is a surgical approach that can be an excellent alternative to a median sternotomy. However, for aortic root interventions, the median sternotomy is still the gold standard. Most surgeons are slow to master this method because the visualization is poor and less convenient.

Total Arch Replacement Versus Hemiarch Replacement in Hemodynamic Performance: A Simulation Study

International Journal for Numerical Methods in Biomedical Engineering
ABSTRACTAcute type A aortic dissection (ATAAD) is a life‐threatening aortic emergency with high mortality. Currently, hemiarch replacement (HAR) and total arch replacement (TAR) are the primary surgeries for ATAAD, but their long‐term outcomes remain debated, possibly due to the influence of clinical factors in multicenter studies.

Validity of a Limited Ascending and Hemiarch Replacement for Acute Type A Aortic Dissection

The Annals of Thoracic Surgery, 2006
The extent of arch repair at emergency surgery for acute type A dissection is controversial. This study was designed to evaluate the rationale of tear-oriented conservative ascending/hemiarch replacement, comparing it against total arch replacement.A total of 134 consecutive patients with acute type A dissection who underwent emergency surgery between ...
