2 cases of ictal hemiparesis in adults are presented. The clinical picture was recurrent transient hemiparesis with contralateral focal hemispheric discharge. The etiology in such cases seems to be a stimulation of the second somatosensory and/or the supplementary motor areas.

Left hemiparesis and left-sided ataxia developed in a 67-year-old man. Computed tomography (CT) showed an area of decreased attenuation in the anterior part of the rostral posterior limb of the right internal capsule that enhanced with contrast. As the patient's condition improved, the contrast enhancement resolved.

Right hemiparesis with right-sided pain and ataxia developed in a 68-year-old man. Sensation, neuropsychological function, and somatosensory evoked potentials were normal. Computed tomography showed an isolated fresh infarct in the left part of the thalamus.

Three stroke patients showed weakness and pyramidal signs on one side combined with a cerebellar-like ataxia on the same side. Pathologic study in each case showed an old infarct cavity in the basis pontis at the level of the junction of the upper one third and lower two thirds on the side opposite the neurologic deficit.

Ictal asomatognosia with hemiparesis
Neurology, 1998A 69-year-old woman presented with an ictal Anton-Babinski syndrome (asomatognosia with hemiparesis). Except for head and eye deviation to the side of the paralyzed limb, epileptic nystagmus, brief episodes of impaired consciousness, and automatisms, clinical symptomatology was identical to Anton-Babinski syndrome of vascular origin.
