The number of patients requiring dialysis is increasing, in particular those patients over the age of 75. The arteriovenous fistula is the preferred access for hemodialysis due to fewer complications and decreased mortality. Access complications are common and require early recognition and treatment.

Hemodialysis is principally used for the management of acute and chronic renal failure that is refractory to conventional medical therapy.

Abstract: As a busy dialysis and apheresis unit and a referral center for vascular access, we had 850 hemodialysis catheter insertions in 2004, and >16 000 since 1976. According to data from literature and our experience, insertion should be guided by real‐time ultrasonography whenever possible.

In collaboration with Christopher R. Blagg at the University of Washington and John F. Sullivan at Cornell College of Medicine, we have conducted a trial of travel dialysis using the Friedman-Hutchisson suitcase system. A total of 122 location dialyses were performed by 18 patients on 22 trips in Europe, Canada and the United States.

The interpretation of traditional serum urea and creatinine concentrations as indices of the severity of uremia requires major modifications in hemodialyzed patients.
