In a PSG‐confirmed OSAS cohort, ePLAR and epicardial adipose tissue (EAT) were higher than in PSG‐normal controls and increased stepwise with disease severity. Combined assessment of ePLAR and EAT showed excellent discrimination for severe OSAS and may support noninvasive risk stratification.
