Obturator hernia is an uncommon problem that occurs predominantly in elderly, debilitated women. The diagnosis should be suspected in such patients who have evidence of small bowel obstruction, often initially intermittent, and who complain of pain along the anteromedial aspect of the thigh.

Herniation of intra-abdominal structures through the obturator foramen in the innominate bone has been recognized as a surgical condition since Arnaud de Ronsil 1 reported the first such incidence before the Royal Academy of Sciences in Paris in 1724. Watson, 2 in 1938, collected 420 cases from the literature.

In the past 15 years at the Mayo Clinic, eight patients underwent nine operations for repair of 11 obturator hernias, which represented 0.073 per cent (11 of 15,098) of all hernias repaired at this institution. Elderly women with chronic disease were most frequently affected.
