ABSTRACT Examination of a first‐time syncope in a 76‐year‐old man revealed a large, space‐occupying arachnoid cyst almost completely occupying the left frontal, parietal, and temporal lobes and showing a midline shift to the right. Syncope can be the initial clinical manifestation of a space‐occupying arachnoid cyst.
