Synaptic ultrastructural alterations in human focal cortical dysplasia: Insights from volume electron microscopy

open access: yesEpilepsia, EarlyView.
Abstract Objective Focal cortical dysplasia (FCD) is a developmental malformation of the cerebral cortex and a leading cause of drug‐resistant epilepsy in children and young adults. Disruption of the excitation–inhibition (E–I) balance is a hallmark of neuronal hyperexcitability in FCD, yet the underlying synaptic ultrastructural changes remain poorly ...

Digital Response Test in Epilepsy assesses interictal epileptiform discharge effects in real time

open access: yesEpilepsia, EarlyView.
Abstract Objective Interictal epileptiform discharges (IEDs) in people with epilepsy (PWE) can impair cognitive functions and increase reaction time (RT) and the likelihood of missed reactions. These effects are not routinely assessed, because reliable methods for detecting IEDs of variable appearance in real time and suitable tests to measure IED ...

Potassium–chloride cotransporter 2 activity dampens induced ictal‐like activity in neocortical slices containing the seizure propagation zone of temporal lobe epilepsy patients

open access: yesEpilepsia, EarlyView.
The K⁺/Cl− cotransporter (KCC2) limits induced ictal‐like events and their spread in the supragranular layer of human neocortical slices from the seizure‐propagation zone of temporal lobe epilepsy patients. Cl⁻ extrusion is present in supragranular pyramidal neurons but limited under high somatic Cl⁻ load.

Seizure classification using a multimodal seizure monitoring system (Nelli) in Dravet and Lennox–Gastaut syndromes: A non‐randomized, single‐center feasibility study

open access: yesEpilepsia, EarlyView.
Abstract Objective This study aimed to assess the performance of the Nelli seizure monitoring system in detecting and classifying seizures during sleep or while at rest in bed in patients with Lennox–Gastaut syndrome (LGS) and Dravet syndrome (DS). Methods We conducted a non‐interventional, single‐center feasibility study from August 2023 to March 2024,
