ABSTRACT Background Patients with hypertrophic obstructive cardiomyopathy (HOCM) were excluded from all major trials for transcatheter aortic valve replacement (TAVR). “Suicide left ventricle” occurring after TAVR is postulated to occur as a result of chronic pressure overload from fixed obstruction that is acutely relieved after the valve deployment ...

Pitfalls of anesthetic management with the Impella® 5.0 device: a case series
Background Impella® is an antegrade left ventricular assist device with a pump catheter in the left ventricle. We report three cases in which we experienced some pitfalls with circulatory management during Impella placement due to new-onset aortic ...

Applying DanGer Shock Eligibility Criteria to a Real‐World Cohort of Cardiogenic Shock Patients
ABSTRACT Background The DanGer Shock trial investigated the effects of adding a microaxial flow pump to guideline‐recommended therapy in patients with ST‐elevation myocardial infarction (STEMI)‐related cardiogenic shock (CS), finding a lower all‐cause mortality at 180 days in the intervention group.

Ventricular tachycardia in ischemic cardiomyopathy; a combined endo-epicardial ablation as the first procedure versus a stepwise approach (EPILOGUE) - study protocol for a randomized controlled trial [PDF]
Background: The role of epicardial substrate ablation of ventricular tachycardia (VT) as a first-line approach in patients with ischemic heart disease is not clearly defined.

We experienced a case of intraoperative coronary artery dissection that became apparent during weaning from cardiopulmonary bypass. This case demonstrates the successful management of cardiogenic shock secondary to intraoperative left coronary artery dissection using venoarterial extracorporeal membrane oxygenation for beating‐heart coronary artery ...

Percutaneous Salvage of an Impella Pretzel
This report describes a case in which bedside manipulation of an Impella CP led to device entanglement in the aorta. Using a snare and counter-traction, we were able to untangle and re-deliver it into the left ventricle to restore normal device support. (Level of Difficulty: Advanced.).

Impact of standardized team‐based care on cardiogenic shock outcomes over time
This contemporary North American cardiogenic shock (CS) registry aimed to investigate the influence of team‐based care on outcomes over a six‐year period of time following implementation of standardized protocols. Despite a two‐fold increase in the number of CS patients and changes in presentation and management strategies, no differences in in ...

Acute myocardial infarction complicated by cardiogenic shock: an algorithm based ECMO program can improve clinical outcomes. [PDF]
Objective: Extracorporeal membrane oxygenation (ECMO) in our institution resulted in near total mortality prior to the establishment of an algorithm-based program in July 2010. We hypothesized that an algorithm based ECMO program improves the outcome of
