Inappropriate shocks following recent generator replacement

In a 64-year-old male patient with an implantable cardioverter defibrillator (ICD) for secondary prevention of sudden cardiac death, a shock occurred some months after generator exchange and downgrade to a single-chamber system. Careful analysis of the stored electrogram revealed a rare cause that could be confirmed at the time of operative revision.

A 53‐year‐old man with nonischemic cardiomyopathy underwent implantation of a biventricular implantable cardioverter defibrillator (ICD) for symptomatic ventricular tachycardia. He received five shocks while attempting to exercise, 48 hours after implantation.

Appropriate Result from an Inappropriate ICD Shock

A case of inappropriate detection of an ungrounded external AC current by an implantable cardioverter defibrillator (ICD) is reported. The resultant ICD shock dislodged the patient from the electrical source, thus preventing injury or electrocution.
