Shortening surgical training through robotics: randomized clinical trial of laparoscopic versus robotic surgical learning curves

open access: yesBJS Open, Volume 4, Issue 6, Page 1100-1108, December 2020., 2020
Medical students and surgical trainees were randomized to robotic or laparoscopic simulator training followed by cadaveric surgical tasks. The robotic groups had a shorter learning curve with better task performance. Another advantage for robotic surgery Background Minimally invasive surgery is the standard technique for many operations.

Variation in descriptors of patient characteristics in randomized clinical trials of peptic ulcer repair: a systematic review

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 12, Page 1570-1579, November 2020., 2020
This study demonstrates the variation in reporting of descriptors of patients in studies comparing laparoscopic and open peptic ulcer repair. It shows wider variation, and proposes the need to address this to improve the quality of research. Makes comparisons difficult Background The ability to compare findings across surgical research is important ...

Breast cancer surgery in older women: outcomes of the Bridging Age Gap in Breast Cancer study

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 11, Page 1468-1479, October 2020., 2020
Age, ill health and frailty all influence surgical decision‐making for breast cancer. Breast cancer surgery is safe, with serious adverse events being rare and no mortality. However, surgery has a negative impact on quality of life and independence, which must be considered when counselling patients about choices.

Decrease in primary but not in secondary abdominal surgery for Crohn's disease: nationwide cohort study, 1990–2014

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 11, Page 1529-1538, October 2020., 2020
In a Swedish population‐based database, the cumulative incidence of a first abdominal surgical procedure after a diagnosis of Crohn's disease has decreased by two‐thirds over the past 25 years. The risk of repeat abdominal surgery has not decreased since the introduction of biologicals. Ileocaecal resection remained the most common procedure during the

Effect of intraoperative nerve monitoring on postoperative vocal cord palsy rates after thyroidectomy: European multicentre registry‐based study

open access: yesBJS Open, Volume 4, Issue 5, Page 821-829, October 2020., 2020
The influence of intraoperative nerve monitoring (IONM) and other factors on the development of postoperative vocal cord palsy (VCP) was assessed using multivariable regression analysis. In a cohort of 4598 patients from 82 hospitals, IONM was used in over 90 per cent of those treated by primary thyroidectomy for underlying benign thyroid disease. IONM

Early operative management in patients with adhesive small bowel obstruction: population‐based cost analysis

open access: yesBJS Open, Volume 4, Issue 5, Page 914-923, October 2020., 2020
This population‐based cost–utility analysis compared early operative management (EOM) for adhesive small bowel obstruction (SBO) to the current standard of care of a trial of non‐operative management. Although EOM was more costly, it was associated with a significantly lower risk of recurrence and reduced exposure to the morbidity and costs associated ...

Total colonic aganglionosis: multicentre study of surgical treatment and patient‐reported outcomes up to adulthood

open access: yesBJS Open, Volume 4, Issue 5, Page 943-953, October 2020., 2020
In a multicentre study, outcome of surgery for total colonic aganglionosis was evaluated in 116 patients. In long‐term follow‐up, patients with a J pouch reconstruction reported better faecal continence and those with a Duhamel reconstruction reported fewer episodes of enterocolitis. Nutritional support was needed extensively in all groups with various

Surgical training rotation design: effects of hospital type, rotation theme and duration

open access: yesBJS Open, Volume 4, Issue 5, Page 970-976, October 2020., 2020
This study aimed to assess whether hospital status, rurality, duration of hospital rotations and specialty theme of core surgical training had an impact on workplace productivity. Longer placements improve outcomes Background Entrants into UK surgical specialty training undertake a 2‐year programme of core surgical training, rotating through ...
