A Case of Abscess of the Gastric Wall Associated with Intraabdominal Abscess.

腹腔内膿瘍をきたした胃壁膿瘍の1例を経験したので報告する.症例は73歳,女性.腹痛・全身倦怠を主訴に受診,エコーにて胆嚢の腫大を指摘され,急性胆嚢炎として入院し保存的に加療された.症状は一時改善したが,発熱とCRPの上昇を認め, CT上胆嚢腫大と肝左葉背側,脾背側,胃後壁に多量の腹水の貯留を認め腹腔内膿瘍の診断となった.そこでPTGBD (E. aerogenesが検出)および腹腔内膿瘍ドレナージ(K. oxytoca & P. aeruginosaが検出)を施行した.ドレナージにより膿性腹水は減少し手術となった.術中所見で胃体下部後壁粘膜下に8×6cm大の粘膜下腫瘍類似病変を認め,幽門側胃切除・横行結腸部分切除を行った.病変内部より腹腔内膿瘍と同様の膿汁が採取され培養ではK ...
