Intravenous leiomyomatosis extending to the heart

Intravascular leiomyomatosis is a rare uterine tumor characterized by the growth of benign smooth muscle tumor into venous channels or lymphatics. These arise from a uterine myoma. Although benign, they may mechanically obstruct the inferior vena cava, right heart or the pulmonary artery resulting occasionally in death w1x.

Intravenous leiomyomatosis with cardiac extension is an extremely rare uterine tumor. We report here a case of a patient with a uterine leiomyoma which extended into the right atrium through the left ovarian vein, progressing into the left renal vein along the inferior vena cava.

Although IVL is histologically benign, it might be aggressive in its behavior and can grow into pelvic veins and the inferior vena cava (IVC) extending into the heart chambers and pulmonary vasculature. Occasionally, it was found to have lung metastasis.

We report a case with intravenous leiomyomatosis of uterus that showed characteristic appearances on MR. Magnetic resonance demonstrated a tangle of distended and folded tubular structures, well enhanced by Gd-diethylenetriamine pentaacetic acid.

She was successfully treated with myomatectomy (left renal vein and inferior vena cava), hysterectomy, and bilateral salpingo-oophorectomy under the cardiopulmonary bypass.

The successful treatment consisted of abdominal hysterectomy, bilateral salpingo-oophorectomy, venotomy and extraction of 3 cm long mobile tumour from the right common iliac vein.

[Intravenous leiomyomatosis].

A 49-year-old woman presented with a uterine tumour with solid and cystic parts. Extension of the tumour into the broad ligament was noted at the operation with wormlike plugs of tumour within the veins. The histological diagnosis of intravenous leiomyomatosis was evident.
