In this review, we discuss the scientific basis for how neurocognitive aging occurs after critical illness, including sepsis, trauma, and burns, and the clinical need to develop therapies to reduce inflammation, protect the blood–brain barrier, remove senescent cells, and enhance neuroplasticity.

A Case of Japanese Spotted Fever with Extreme Vasculitis
49歳の男性, 千葉県在住。初診の10日前に養鶏場の大掃除をした。7日前より38℃以上の発熱と全身の紅斑が認められ, 当科受診した。刺し口を疑う皮疹は認められなかったが, 高熱が続き, 腎機能障害も認められたため, リケッチア症の可能性を考え, 塩酸ミノサイクリン200mg/dayを内服したところ速やかに解熱した。日本紅斑熱抗原に対する抗体価はIgM, IgGともに160倍以上に上昇しており, 日本紅斑熱と診断した。紅斑部の病理組織では, 真皮上層から下層の血管に白血球破砕性血管炎を認めた。病理組織を検討した報告は我々が調べ得た限り本邦で2例のみであった。我々は, 皮膚生検で白血球破砕性血管炎を認め, 腎機能障害を併発した日本紅斑熱の1例を経験したので, 若干の文献的考察を加え報告した。
