This report details the presentation, workup, and treatment of the largest lacrimal duct cyst ever reported in the ophthalmic literature. Included in this report is the first computed tomographic (CT) image of this unusual lesion. Typical and atypical clinical features of this lacrimal sac cyst are emphasized.

Lacrimal Sac Tumors

This is a clinicopathologic study of 115 lacrimal sac neoplasms in adults (mean 52 years). The most common presenting signs and symptoms were epiphora (53%), recurrent dacryocystitis (38%), and/or lacrimal sac mass (36%). The tumors were divided into epithelial (82 cases) and nonepithelial (33 cases) neoplasms.

Lacrimal Sac Septum

A right orbit was exenterated from a male cadaver that was 85 years of age at death. It was microscopically shown for the first time that the lacrimal sac septum divided the lumen into two spaces. It consisted of fibrous tissue with a cavernous structure and was lined with stratified columnar epithelium similar to the lacrimal sac wall.

Lacrimal Sac Melanoma

A 48-year-old woman presented with a 5-month history of unilateral epiphora and progressive growth of a left-sided medial canthus mass. CT revealed a nonenhancing soft tissue mass of the left lacrimal sac with widening and erosion of the nasolacrimal canal. The tumor was resected and pathologically confirmed to be malignant melanoma.

An 84-year-old Caucasian lady was referred to the clinic with a 4-month history of a non-tender right medial canthal swelling (figure 1). This was associated with initial soreness and discharge only. There was no associated epiphora. Her systemic medications included aspirin and calcium supplements. Visual acuity was 6/7.5 in each eye.

Congenital lacrimal sac diverticulum

We report the case of a 6-month-old boy with recurrent episodes of acute dacryocystitis from age 3 months to 1 year of age. The cause was determined to be congenital nasolacrimal sac diverticulum, a rare and commonly asymptomatic entity. An isolated diverticulectomy without dacryocystorhinostomy was performed because of lacrimal drainage system patency.
