While several oncogenic pathogens cause site‐specific cancers, uncertainties remain about many other chronic infections and combined pathogen effects, especially in non‐Western populations. A 65‐year‐old patient developed acute hemolytic transfusion reaction (AHTR) after receiving P1‐positive red blood cells. The causative anti‐P1 antibody was detectable only at 4°C, evading routine 37°C pre‐transfusion screening.
