Abdominoperineal resection (APR) after minimally invasive rectal cancer surgery was associated with significantly higher rates of erectile and early ejaculatory dysfunction compared with anal‐sparing surgery. Abstract Aim Lymph node metastases occur in 10%–15% of high‐risk T1 rectal tumours. Total mesorectal excision remains the standard treatment, whereas chemoradiotherapy is reserved for patients with frailty. Indocyanine green (ICG)‐guided lymphadenectomy may represent an organ‐preserving alternative for patients at high risk of node metastasis.
