Liposarcoma is second in frequency only to malignant fibrous histiocytoma among the soft-tissue sarcomas. It occurs almost exclusively in adults and is found most often in the thigh or retroperitoneum. It rarely arises from a lipoma and does not occur in the subcutaneous tissues.

A clinically appearing, well‐encapsulated lipoma was locally excised from the right posterior upper buccal gingival sulcus of a 24‐year‐old man and diagnosed histologically as pleomorphic liposarcoma. A right maxillectomy with preservation of orbital function was followed by 5,000 rads of planned postoperative radiation therapy.

The diagnosis of retroperitoneal fatty tumour is not extremely difficult provided that the possibility of its presence is kept in mind by the surgeon in all cases of distended abdomen in an apparently well‐nourished patient. A careful clinical examination and a good excretory Myelogram arc very important in the diagnosis.
