High-Flow Nasal Cannula Versus Conventional Oxygen Therapy in Patients Undergoing Thoracic Surgery: A Randomized Controlled Trial. [PDF]

open access: yesThorac Cancer
Short‐term perioperative high‐flow nasal cannula (HFNC) oxygen therapy, applied during anesthetic induction and for 30 min post‐extubation, did not significantly reduce postoperative pulmonary complications compared to conventional oxygen therapy in patients undergoing elective thoracic lung resection surgery.

Developmental outcomes after early surgery for complex congenital heart disease: a systematic review and meta‐analysis

open access: yesDevelopmental Medicine &Child Neurology, Volume 63, Issue 1, Page 29-46, January 2021., 2021
Aim (1) To systematically review the literature on developmental outcomes from infancy to adolescence of children with complex congenital heart disease (CHD) who underwent early surgery; (2) to run a meta‐regression analysis on the Bayley Scales of Infant Development, Second Edition Mental Developmental Index and Psychomotor Developmental Index (PDI ...

Selective decontamination of the digestive tract in upper gastrointestinal surgery: systematic review with meta‐analysis of randomized clinical trials

open access: yesBJS Open, Volume 4, Issue 6, Page 1015-1021, December 2020., 2020
This meta‐analysis investigated the influence of selective decontamination of the digestive tract (SDD) on outcomes after upper gastrointestinal surgery. SDD led to a reduction in anastomotic leakage (odds ratio (OR) 0·39, 95 per cent c.i. 0·19 to 0·80; P = 0·010) and pneumonia (OR 0·42, 0·23 to 0·78; P = 0·006).

Alterations in blood microbiota after colonic cancer surgery

open access: yesBJS Open, Volume 4, Issue 6, Page 1227-1237, December 2020., 2020
This study investigated changes in the amount of bacterial DNA in blood and the diversity of blood microbiota in the perioperative period in 30 patients undergoing minimally invasive surgery for colonic cancer. DNA encoding the bacterial 16S ribosomal RNA gene (16S rDNA) was found in whole blood from these patients, and the amount of bacterial 16S rDNA

Laparoscopic versus open subtotal gastrectomy for gastric adenocarcinoma: cost‐effectiveness analysis

open access: yesBJS Open, Volume 4, Issue 5, Page 830-839, October 2020., 2020
Laparoscopic subtotal gastrectomy (LSG) is associated with good perioperative outcomes and superior quality of life compared with the open approach to resection of gastric adenocarcinoma, albeit at higher cost. A cost–effectiveness analysis comparing the two approaches found LSG to be cost‐effective compared with the open approach, with an incremental ...

Minimally invasive techniques for transthoracic oesophagectomy for oesophageal cancer: systematic review and network meta‐analysis

open access: yesBJS Open, Volume 4, Issue 5, Page 787-803, October 2020., 2020
Minimally invasive and robotic techniques for oesophagectomy are associated with reduced perioperative morbidity and length of hospital stay, with no compromise of oncological outcomes but no improvement in perioperative mortality. There are suggestions of improved long‐term survival with minimally invasive oesophagectomy and robotic minimally invasive

Immediate and long‐term impact of the COVID‐19 pandemic on delivery of surgical services

open access: yesBJS (British Journal of Surgery), Volume 107, Issue 10, Page 1250-1261, September 2020., 2020
Surgical services are adapting to mitigate the surge in patients with COVID‐19 in need of critical care support. All non‐essential elective surgery has been cancelled, or is pending cancellation, in healthcare systems around the globe, impacting millions of patients.

Long‐term outcomes after perioperative treatment with omega‐3 fatty acid supplements in colorectal cancer

open access: yesBJS Open, Volume 4, Issue 4, Page 678-684, August 2020., 2020
Perioperative treatment with omega‐3 fatty acids (n‐3 FA) does not confer an overall survival benefit in patients undergoing colorectal cancer surgery, and neither is it associated with an increased efficacy of adjuvant chemotherapy. Clinicians should be careful when making any recommendations regarding the use of n‐3 FA oral nutritional supplements ...

Outcomes for high‐risk hepatoblastoma in a resource‐challenged setting

open access: yesBJS Open, Volume 4, Issue 4, Page 630-636, August 2020., 2020
The outcomes of children with high‐risk hepatoblastoma were assessed in the setting of a resource‐challenged nation, where access to organized chemotherapy and liver transplantation remains limited for the underprivileged. Excellent results were demonstrated.

Anastomotic techniques for oesophagectomy for malignancy: systematic review and network meta‐analysis

open access: yesBJS Open, Volume 4, Issue 4, Page 563-576, August 2020., 2020
Stapled anastomoses were associated with lower anastomotic leak rates than hand‐sewn anastomoses following oesophagectomy. Furthermore, linear stapled/semimechanical (LSSM) techniques were associated with lower rates of anastomotic stricture than circular stapled or hand‐sewn anastomoses.
