69歳,女性. 1989年4月末より食欲不振,全身倦怠感,発熱が出現.近医でLDH高値,汎血球減少を指摘され精査加療目的で当科入院.入院時より発熱,貧血,扁桃腫大,胸骨叩打痛,下腿浮腫を認め,その後急速な頚部リンパ節の腫脹,肝脾腫が見られDICを併発した.骨髄穿刺を行い, N/C比の高いperoxidase, esterase反応陰性の組織球様の異型細胞を7.2%に認め,同細胞のerythrophagocytosisの所見および臨床所見よりmalignant histiocytosisと診断した.骨髄生検の免疫組織化学的検索でS-100蛋白, α1-antitrypsin染色はともに陰性であった.
