Massive hemoptysis due to broncholithiasis

Massive hemoptysis due to broncholithiasis is rare. Such a case is presented here, and the literature is reviewed. Surgical resection is the preferred definitive therapy, as a lack of bronchial artery collaterals limits the utility of bronchial artery embolization.

A 53-year-old woman presented with a productive cough, fever, chills, and night sweats of one month's duration. She reported having had lightly blood-streaked sputum initially but then experiencing massive hemoptysis (> 200 mL/2 hr). Since the onset of symptoms, she had had malaise, body aches, and a 27-lb weight loss.

Operative Treatment of Massive Hemoptysis

Abstract Fifty-five pulmonary resections have been performed at our institution for hemoptysis of 600 ml. or more in 24 hours. The overall mortality was 18% as compared with more than 75% in patients who bled 600 ml. or more in 16 hours and 54% in patients who bled 600 ml. or more in 48 hours, all of whom were managed conservatively.

Massive hemoptysis in cystic fibrosis

Massive hemoptysis is occurring with increasing frequency in older patients with cystic fibrosis; it signifies a poor prognosis. Twenty-nine episodes occurring in 19 patients are analyzed. Thirteen of the 19 patients died within 6 months of the initial episode. No single aspect of the pulmonary treatment program has been incriminated.

A 39-year-old African American man presented with massive hemoptysis. He was known to have a history of type III sarcoidosis and a chest x-ray consistent with aspergilloma. Bronchoscopy was performed within 48 hours from the onset of the patient's symptoms revealing generalized heme in both the left and right main bronchi.
