Tumors of the mediastinum represent a wide diversity of disease states. The location and composition of a mass is critical to narrowing the differential diagnosis. The most common causes of an anterior mediastinal mass include the following: thymoma; teratoma; thyroid disease; and lymphoma.

Teratomas of the mediastinum

Mediastinal teratomas are rare, constituting only 8-13% of tumours in this region. Teratomas often contain structures from all three germ layers, and 15-20% are malignant. A report is presented of eight patients operated on in 1978-1985 for mediastinal teratoma histologically classified as benign.

1 A brief review of the literature on mediastinal tumors is offered, with special attention to fibrosarcoma of the mediastinum. 2 Three unusual cases of mediastinal fibrosarcoma are reported. 3 Two of these showed a remarkable degree of mediastinal compression. Superior vena cava obstruction was extreme and pulmonary insufficiency intractable.

Pseudotumor of the Mediastinum

The case of a mass in the mediastinum which occurred following surgery and irradiation at the site of previously excised thymoma is presented. This mass proved to be a pseudotumor consisting of fibrous tissue without evidence of tumor recurrence.

A case of leiomyosarcoma of the posterior mediastinum is presented; 2 other malignant and 7 benign smooth muscle tumours of the mediastinum have been collected from the literautre. The tendency for blood-borne and local recurrence in the leiomyosarcomas is demonstrated. Various theories for the origin of these tumours are examined.
