ABSTRACT An adrenal crisis should be considered in children with a history of tuberculosis who present with shock, hypoglycemia, and hyperpigmentation. Even years after successful treatment, TB can cause irreversible adrenal damage leading to primary adrenal insufficiency. Early recognition and prompt glucocorticoid replacement are lifesaving.
