Migraine equivalents and complicated migraine are entities in which definition is difficult, presentations are pleomorphic, diagnosis is treacherous, pathophysiology is obscure, and treatment is uncertain. A useful principle is to regard them as diagnoses of exclusion that require aggressive and comprehensive investigation.
