Neurologists should be able to identify clinical and neuroimaging features that distinguish vascular disorders from other causes of myelopathy.Although certain clinical features suggest a vascular etiology in acute and chronic myelopathy settings, accurate MRI interpretation within the clinical context is key.

Myelopathy is an inclusive term, referring to pathology leading to a neurologic deficit related to the spinal cord. The clinical diagnosis of myelopathy requires a detailed history and physical examination to define the clinical syndrome. Neuroimaging is indicated in most instances of new-onset myelopathy.

Myelitis and myelopathy may be caused by numerous ideologies, such as neoplastic, inflammatory, metabolic, infectious, and physical agents. Cervical spondylitic myelopathy, a spectrum of degenerative change occurring in the cervical spine, is the most common cause of cord dysfunction in patients more than 50 years of age.

Irradiation treatment in the cervical and thoracic areas is sometimes accompanied by the development of neurological symptoms and signs. The clinical course and the diagnostic difficulties encountered in such cases are illustrated in four patients. Special attention is given to those rare post-mastectomy cases.

Postshunt myelopathy is a rare, severely debilitating syndrome complicating portosystemic shunts that is characterized by spastic paraparesis. In this report we describe a 39-year-old woman with alcoholic liver disease and postshunt myelopathy resulting in an inability to walk.

Although infectious myelopathies are rare, timely and accurate diagnosis is essential to improving outcome. There are a number of organisms that may cause infectious myelopathies, including human immunodeficiency virus (HIV), human T-cell lymphotropic virus type I (HTLV-I), herpesviruses, enteroviruses, Treponema pallidum, Mycobacterium tuberculosis ...

Myelopathy is commonly encountered in clinical practice and is associated with a large number of causes. This article reviews the anatomy of the spinal cord and discusses how the clinical findings, time course, and radiographic patterns can help to identify the causes of myelopathy.Imaging observations such as MRI "pancake" gadolinium enhancement ...
