Electrophysiological studies of myoclonic jerks provoked by photic stimulation

A patient with a type of myoclonus produced by somatosensory and photic stimuli was studied electrophysiologically. Jerk-locked averaging showed cortical spikes in association with the spontaneous myoclonus. High amplitude somatosensory evoked potentials and high amplitude visual evoked potentials were observed.

Abstract In rats deeply anaesthetized with urethane, injection of 1,2 dihydroxybenzene (catechol; 60 mg/kg i.p.) produces a stimulus-sensitive state in which a variety of sensory stimuli, e.g., a brief electrical shock or mechanical tap applied to the periphery, or a binaural click, produce brief (myoclonic) muscular jerks.

Previous studies in our laboratory have demonstrated that microinjection of N-methyl-D-aspartate (NMDA) agonist into the nucleus magnocellularis (NMC) of the medial medulla increases muscle tone and/or produces locomotion, while injection of corticotropin-releasing factor (CRF) and non-NMDA agonists into the same or nearby sites suppresses muscle tone.

Eleven of 40 decerebrated cats were found to exhibit periods of spontaneous or sensory myoclonus and locomotion beginning 24 h after decerebration. Histological analysis showed that the cats generating myoclonus hemorrhagic lesions in the retrorubral nucleus (RRN) and ventral mesopontine junction (vMPJ).

The mechanism of cerebral hypoxia-induced myoclonic jerks is not known. Some studies have suggested that glutaminergic NMDA receptor activation in the inferior olive resulting in excitotoxic neuronal injury in the cerebellum is the underlying cause of posthypoxic myoclonus.
