Percutaneous nephrolithotomy in the elderly.
1982年5月以来172例に対し, 経皮的腎尿管結石摘出術を施行し, 60歳以上の症例は14例であった. 60歳未満の症例と比較し高齢者に対して本法を施行するうえでの問題点を検討した. 硬膜外麻酔下に患者をレントゲン透視台に腹臥位にねかせる. 側背部から腎盂まで経皮的に腎瘻を造設し, 内視鏡を挿入し結石を分割, 摘出する.対象となった患者は男性121名, 女性51名, 性比2.4:1であった. 60歳以上の症例では, 男性9名, 女性5名, 性比1.8:1とやや女性の比率が増加していた. 14症例のうち1名の尿管結石症例では本法で結石摘出が出来ず, 従来の尿管切石術を施行した. 基礎疾患として高血圧症と心房細動を有していた腎盂結石症例では術後心不全にて死亡した. 5mm以上の残石を認めた症例は3例 (
