Non-Neoplastic Mediastinal Cysts
Advances in Anatomic Pathology, 2020Primary mediastinal cysts are infrequent lesions that can arise from a variety of mediastinal organs or structures. Most of these are congenital in origin and incidental findings during investigations for unrelated conditions. We identified 53 patients with non-neoplastic cysts of the pineal gland. In contrast to patients with pineal neoplasms, pineal cysts are usually asymptomatic. They infrequently obstruct the aqueduct to cause hydrocephalus or compress the tectum to produce the neuro-ophthalmologic signs of dorsal midbrain dysfunction.

The cytologic method is a very useful procedure during intraoperative consultation of non-neoplastic disorders, because the features of inflammatory cells and macrophages, obscured in frozen sections, are nicely preserved in smears. The presence of large number of inflammatory cells or abundant macrophages virtually rules out the diagnosis of neoplasia

The category “negative for intraepithelial lesion or malignancy” is used for specimens that show a spectrum of nonneoplastic changes, including those associated with protective and reactive responses to inflammation, hormonal alterations, and colonizing or infectious organisms. Recognition of the wide variety of normal cellular presentations is key for
