Although tibia metaphyseal nonunion is rare, its treatment is often complex. The merits of related management techniques are discussed. These techniques include: intramedullary nailing, fine wire fixation, and blade plate reconstruction, which is the method preferred by the authors.

Twenty-eight patients with metaphyseal nonunion were evaluated retrospectively. Initially, all patients were thought to have healing of their fractures. Mild pain and/or instability with weightbearing or discomfort localized around the hardware were common symptoms of this nonunion; however, substantial pain was not a predominant feature.

Nonunions of long bone fractures can be treated successfully with one operative procedure in more than 90% of patients. In fact, 80% of patients can have good to excellent final restoration of mechanical axis alignment and proper length. Patients with infected nonunions may require more than one procedure to eliminate infection and heal the nonunion ...

Treatment of Noninfected Nonunions: Normotrophic Nonunions
2006In normotrophic nonunions (the intermediate category between hypertrophic and atrophic nonunions), vascularization at the nonunion site is poor but exists, and on the bone scan detection of the tracer is poor. The biological activity of the connective tissue in the interfragmentary gap is low but not absent.
