CASE REPORT OF INFECTION BY AN OCCULT HBV CARRIER
2004年10月に医療機関から輸血後HBV感染が疑われる症例が報告された. この患者に輸血された33本全てについて個別核酸増幅検査 (個別NAT) を行ったところ, 一人の献血者から得られた血液が陽性と確認され, 患者と当該血液のHBV-DNA sequence はほぼ一致し, 因果関係が認められた. この献血者は, 54歳男性で過去11年間に78回の献血歴があり, HBs抗原陰性で個別NATが所々で陽性となる occult HBV carrier であった. 同献血者の血液は, 過去3回, 輸血後HBV感染の疑いとして報告された患者に使用されていた. 報告を受けた時点では, 個別NATが全て陰性であったため, 感染の因果関係が立証できなかった.

Subacute Thyroiditis Without Neck Pain: An Unusual Cause of Persistent Fever in the Elderly Patient
ABSTRACT Subacute thyroiditis can present as fever of unknown origin, particularly in elderly patients. Absence of neck pain or thyroid tenderness does not exclude the diagnosis. Thyroid function tests and inflammatory markers are key to diagnosis. Corticosteroids lead to rapid symptomatic improvement in moderate‐to‐severe cases.
