Clinical Case Reports, Volume 14, Issue 7, July 2026.ABSTRACT A 35‐year‐old woman with recurrent neurogenic thoracic outlet syndrome underwent supraclavicular neurolysis and brachial plexus wrapping using a free DIEP flap. At 2‐year follow‐up, pain and functional scores (VAS, DASH, SF‐36) significantly improved. Free fat flaps offer a low morbidity option for reducing perineural fibrosis in RNTOS.
