A prospective study to evaluate discharge of patients from the hospital the day of open cholecystectomy was performed. Patients were selected for outpatient operation if they were less than 55 years of age, did not undergo exploration of the common bile duct and had no significant co-morbidity.

This report deals with the first 350 laparoscopic cholecystectomies performed at Creighton University, beginning in October 1989, by one of the authors (R.J.F., Jr.). An open technique was used for initial access to the abdominal cavity in 343 patients (98%). Patients included 274 females and 76 males, with an average age of 45.5 years.

The experience with "open" biliary tract surgery is documented in this report in an effort to provide a basis for comparison with the more recent experience with laparoscopic cholecystectomy. During the years 1932 through 1984, 14,232 patients were surgically treated for nonmalignant biliary tract disease at The New York Hospital-Cornell Medical Center.
